1. Port Placement and Angle
  • Placing the ports at an optimal angle is crucial for a good working corridor. Typically, one port serves as the viewing port, and the other as the working port. Placing them in a triangulated fashion helps with instrument maneuverability.
  • Ensuring an optimal distance between the ports can also help in minimizing instrument collision, which can make the procedure smoother.
2. Creating a Safe Working Space
  • Clearing soft tissue around the foramen and epidural space with radiofrequency ablation or blunt dissection early on can prevent excessive bleeding and give better visualization.
  • Using specialized instruments like a diamond burr for bony work can allow precise removal of bone while protecting neural elements.
3. Disc Fragment Removal
  • Once the disc space is reached, using pituitary forceps or microforceps to remove herniated fragments is key. Tactile feedback here is important to ensure only the targeted fragments are removed.
  • Gently probing the annulus and ensuring no residual fragments are left can help prevent recurrence.
4. Bleeding Control
  • Bipolar cautery or hemostatic agents help manage any bleeding, especially around the venous plexus, which can sometimes obscure the view.
5. Post-Procedure Verification
  • Once decompression is complete, it can help to confirm the nerve root is free and mobile, particularly in cases of severe compression.
